    With mine, i never check the certificate box. For me i have never had to add credentials (username and password). I usually just add the http:// hit exit and then connect my service. Make sure your box has the correct date and time when you first start the box. If it doesnt that means you are not connected to the internet. but try it again without clicking on the certificate box just leave it empty.

    Would a mismatch MAC address give me "Your account is blocked or expired" ?

    of course any single or tiny mistake with url/mac will give you error. if service is still valid or active and mac/url is correct then try to perform a reset on your provider site and reboot your box, modem, and router.
